CLAIMS: Claims MUST be initiated within 10 days after the coverage end date, NO EXCEPTIONS.
Please email claims@mychbo.com and provide the following:
CHBO property ID -
CHBO Username -
1. Damage Reported On?
2. Who Reported Damage? Owner or Tenant
3. Damage Type: Damage or Theft
4. Amount of Claim? If the amount is not known, please provide an estimate. Payment is based on the Invoice/Receipt amount.
5. Full Address Damage occurred
6. Description of Damage: (Required: Include original purchase date for each damaged item, and make/model for items over $200)
7. Please describe the corrective action being taken?
8. Please describe the corrective action being taken? (This is a reimbursement program. So all services/purchases must be proven with Invoice and Paid Receipts. You CANNOT self service anything and receive a reimbursement.)
9. Additional Comments.
10. Provide before and after images of damage
Please include several images of the current damage and before photos if possible. CHBO will initiate the claim process once we receive your email. If needed, we will contact you directly for additional details.
This is a reimbursement program that pays out ONLY based on replacement of the damaged item(s) only. Labor cost is not covered for items that are serviced or repaired by a non licensed company. CHBO will disburse payment once the claim has been finalized.
